Clinical Intake Agent

reimagine patient intakeClinical Intake Agent

Today’s intake workflows are strained — with overburdened staff, missing vitals, and rushed patient encounters. Our patented, Clinical Intake Agent™ solves these problems by creating an intelligent, sensor-connected intake experience that meets patients where they are.  It works seamlessly in clinics, at home, or in hybrid settings — wherever care begins.

Our patent-pending AI Agents guide patients through an easy, conversational check-in — collecting vitals, symptoms, medications, and reason for visit — validating wirelessly connected sensor readings in real time and delivering structured data directly into the EHR. Your care team gets complete, actionable insights before entering the room — reducing delays, boosting efficiency, and improving patient satisfaction.

Supports Care Anywhere

The Clinical Intake Agent is designed to work wherever patients are — in clinic, at home, or as part of remote care programs. It seamlessly supports:

  • Clinical Intake in the Exam Room – Self-administered vitals and intake before the provider enters

  • Telehealth & Virtual Care – Guided, structured intake before or during virtual visits

  • Remote Monitoring & Chronic Care – Vitals capture and follow-up from home

  • Hybrid & Home-Based Models – Pre-visit triage and post-visit symptom tracking

  • Decentralized Clinical Trials – Real-time, AI-guided symptom reporting and sensor-based data collection from remote participants

By meeting patients in their real-world context, the agent reduces burden on staff while ensuring high-quality, validated intake data flows to the care team.

Demo Experiences

These demos showcase how our conversational AI agents perform specific, real-world tasks — like vitals collection — through natural, intelligent dialogue. What you’re seeing is more than a chatbot. Each interaction is driven by structured prompts, built-in safeguards, and a modular sensing framework that enables real-time data collection.

While the primary demo features the Clinical Intake Agent — our flagship application for guiding patients through intake and vitals collection — we’ve also included examples of how the same AI agent framework can support additional task execution. These include routine check-ins, threshold-based alerts, post-care follow-ups, and more.

Each demo illustrates how our AI agents can be configured to collect behavioral and biological data together — and how that unified insight can drive smarter, faster, and more human-centered care. More sensors and tasks can be added to meet your needs, making this platform as simple or as powerful as your use case requires.

Key Features

The Clinical Intake Agent™ is built for fast adoption and future growth. From day one, it delivers intelligent check-ins, real-time vitals validation, and medication review — all through a natural, multilingual conversation powered by persistent memory.

As your organization’s needs evolve, powerful modular extensions can be added seamlessly, expanding capabilities while keeping the patient experience easy, guided, and intuitive.

Core Features (Included from Day One)

Digital Patient Check-In
Patients log in using their smartphone or a provided tablet to confirm demographics and health history — seamlessly linked to their health portal (e.g., MyChart or equivalent).

Multilingual Conversational Interface
Patients select their preferred language at the beginning of the session. The Clinical Intake Agent™ then delivers real-time translation, ensuring all responses are structured and delivered in the provider’s default clinical language.

Guided Vitals Collection with AI Validation
Step-by-step assistance helps patients apply connected clinical-grade devices like blood pressure cuffs, pulse oximeters, thermometers, and scales. Vitals are validated in real time to ensure clinical-grade accuracy.

Medication Review and Reconciliation
The agent guides patients to verify or update medications, syncing changes live with the EHR — supporting safer, more accurate medication reconciliation.

Persistent Memory Across Sessions
The system remembers patient-specific details across visits, allowing for faster, more personalized check-ins without redundant questions.

Optional Extensions (Expand When Ready)

Reason for Visit and Symptom Input
Patients describe why they’re seeking care, with responses structured automatically for clinician review — reducing intake friction.

Digital Intake Forms
Streamline workflows by replacing paper forms with conversational digital capture of allergies, insurance, medical history, and consent.

Standardized Screenings
Easily embed validated screeners like PHQ-9, GAD-7, or custom assessments, with structured results flowing directly into the EHR.

Simulated Clinical Interview
Engages patients through dynamic, physician-style questioning — adapting based on patient responses to surface deeper clinical insights.

Integrated Intake Summary for Clinicians
All collected data — vitals, symptoms, medications, and behavioral insights — is synthesized into a structured summary for the provider, alerting them that the patient is fully intake-ready.

Optional Safety and Intelligence Add-Ons

AI Escalation Triggers
If sensor readings or patient-reported symptoms indicate risk (e.g., abnormal vitals, distress), the agent triggers alerts — even during weekends or after hours.

Patient-Initiated Assistance
Patients can press a “Need Help” button anytime to receive additional guidance from the agent or request staff intervention.

Longitudinal Engagement Layer
The Clinical Intake Agent™ can stay connected between visits — checking symptoms, monitoring adherence, and supporting recovery across time.

How is AI Implemented in the Clinical Intake Agent?

Phase One (Required)

  1. Conversational AI (LLM):  Speaks naturally with patients to collect health info and guide them through intake.

2. Personalization AI:  Adapts session flow based on past interactions and patient preferences.

3. Sensor Validation AI:  Confirms that vitals collected from devices are accurate and properly taken.

Expandable AI Capabilities

1. Behavioral AI:  Monitors patient behavior and tone during intake to detect stress, confusion, or urgency — and adjusts its interaction style or alerts the clinical team.

2. Clinical Triage AI:  Analyzes symptoms and vitals in real time to flag red flags, identify missing data, and help prioritize the case for physician review.

3. Physician Briefing AI:  Prepares a structured, clinically relevant summary of the intake session — highlighting key findings, concerns, and context for the provider.

4. Test Relevance AI:  Identifies potential diagnostic steps — such as lab work or imaging — that may be appropriate based on the intake, without making clinical decisions.

5. Care Escalation AI:  Recognizes when symptoms or vitals suggest a higher level of care may be needed — and alerts staff for immediate follow-up or escalation.

6. AI Coordination Layer:  Integrates with other AI tools used by the clinic (e.g. physician assistants, scheduling bots, documentation systems) to streamline handoffs and care workflows.
